PRE-QUALIFICATION OF SUPPLIERS FOR 2021-2022 FINANCIAL YEAR PROCUREMENT REFERENCE NUMBER ICAP/TZ/03/2021-2022
Introduction:
MSPH Tanzania LLC, known as ICAP is a non-governmental organization operating in Tanzania since 2004, with funding mainly from the Resident’s emergency Flan for AIDS behalf (PEPFAR! through the U.S. Centers for Disease Control and Prevention (CDC). ICAP at Columbia University’s Mailman School of Public Health is the recipient of a five-year PEPFAR funded cooperate agreement beginning 29th September 2021 from the Centers for Disease Control and Prevention [CDC] to support comprehensive community and facility based HIV and VMMC services in Mwanza region
MSPH Tanzania LLC – ICAP Intends to identity and enlist reputable and registered suppliers co various goods works and services for FIKIA + project needs under PEPFAR/ CDC awarded funding.
MSPH Tanzania LLC- ICAP now invites reputable and registered suppliers to be considered as part of its Vendor Register for the period 30th October 2027 to 29th September 2022 for the following lots

| Lot 25 | Supply ,delivery, fixing ,balancing and alignment of motor vehicle tyres |
| Lot 26 | Provision of carbon dioxide gas and cylinder |
QUALIFICATION REQUIREMENTS.
Interested suppliers are advised to include the following in their applications;
1. Brief overview of the company and key staffs (Company profile) including physical business address.
2. Latest Business Registration Certificate or Certificate of Incorporation
3. Latest Copy of TPIN and VAT registration certificates and evidence of tax compliance in Tanzania.
4. Written endorsement of at least three reputable customers dealt with for the past 2 Years other than ICAP Operating Licenses/ Certificates for services or goods which require such.
APPLICATION PROCEDURE
Eligible suppliers meeting all stated requirements are requested to.
a. Submit one application for each lot they are applying for two suppliers is applying for two different lots, each
lot should have a separate application document with supporting evidence specifically for that lot.
b. Interested, eligible suppliers should email their electronic quote via email to procurement department at [email protected]
